Here is some information that will help you complete the forms

OSA Registrant # --- Your membership number is specific to every individual. In the future you'll be able to ask your club for it when necessary. Really you should record this number when you get it and leave it in a safe place. Leave this space blank if you don't have it recorded.

Club Registration # -- Here is the list to date:

Player Classification -- Very Important Section. This Section tells your club how your child or yourself as a player will be registered. Most of the classifications for Northwest Ontario will be MO, YR, or SR. ***Remember if you're still young enough to play youth soccer, but are playing in a senior league, you must register as a senior.

Indoor

• Mini-Indoor - MI

• Youth-Indoor - YI

• Senior-Indoor - SI

• Pro-Indoor - PI

Outdoor

• Mini-Outdoor - MO

• Youth-Competitive - YO

• Senior-Competitive - SC

• Pro-Recreation - PR

Team Registration # -- This number is assigned by your club, it begins with the club registration # then adds digits to make each team specific within the OSA's computer and tracking system, the club should probably post it on the website each year. It would be easiest if the clubs kept the numbers to two digits eg. 01, 02, 03 so on.

League Registration # -- This number is the same as your club number, unless your playing in the Thunder Bay Men's D-League in which case the number is LD-3402-__

Division Registration # -- This number is assigned by the OSA when you begin a club league, it too should be posted by the league/club each year when they set up their league. Each club will be able to see their Division Registration number when they create their divisions and then post it on the website.

Last club played for -- Please don't confuse the TEAM you played on with the CLUB you were affiliated with. For example in the Lake of the Woods Soccer CLUB there was a TEAM called Plaza Fury and in the Borderland Soccer Club there was a TEAM called Gillons.
